For example, you will be asked about the medications you are taking, which specific body parts and functions are affected and about how often you are required to attend medical treatment, as well as the nature of the treatment itself, such as chemotherapy for cancer, dialysis for renal failure, and insulin injections for diabetes cases.
Finally, the Access to Medical Reports section does not contain any questions. Rather, this is a request for medical evidence to back up your answers in the enhanced annuity questionnaire.
